Travel Liability Disclaimer and Waiver
This agreement dated ____________ is between the traveler/travelers,
_______________________________________ (print in your name/s) and Dutch Fork Travel Agency all of its
agents, independent contractors, owner, officers, and all of its representatives and coordinators (now be
referred to as “The Agency”)..
I/We _____________________________, will not hold The Agency responsible in case of: accident, sickness,
loss or damaged luggage and/or any injury, accident or any other loss arising from or out of the bus trip from
Camden or Columbia, SC to and from the Port of Miami and on the Carnival Imagination or any of its activities.
I/We accept that any stops or side trips or any activities associated with this trip I/We are personally responsible
for our own safety during this trip and all of the activities. I/We assume all risks and accept full and complete
responsibilities for any and all damages and personal injury of any kind, including death. I/We as the traveler/s
agree to hold The Agency harmless for any suit, claim, action and causes of action, demand or debt.
Traveler/Travelers can not hold The Agency responsible for “no refund” from the supplier of this vacation/trip
or activities if the traveler/travelers must cancel for any reason. As the supplier: Southern Comfort Bus Lines
and Carnival cruise line does have a “no refund policy” after a certain date and/or severe penalties so as the
traveler I/We accept and understand that without insurance coverage that covers all the aspects of this trip that
I/We may lose all of what I/We have paid or the majority of it.
I/We take full responsibility, for the decision to go on this trip to the Port of Miami and on the above mentioned
cruise and I/We am fully aware of the risk in making this decision. No member of my family, heir or any other
person due to inherit from my death can hold any of the parties above liable or responsible for any of the said
losses. My/Our signature on this form indicates that I/We agree and accept all the terms and conditions for the
said trip on Carnival Imagination and any traveling to and from this cruise.
